Nurse Practitioner (Home Based Primary Care)

The Marion VA Medical Center is recruiting for a Nurse Practitioner (NP) to join our Extended Care Service.
This position is located with the Home Based Primary Care (HBPC) Team at our Community Based Outpatient Clinic in Effingham, IL or Mt.
Vernon, IL.
Duties include, but are not limited to:
oCarrying a case load and making home visits.
oWorking closely with the HBPC Medical Director in the clinical functions of the HBPC Program and being an integral part of an interdisciplinary team.
oPerforming intakes/physical exams.
oDevising patient treatment plans in consultation with other clinicians.
oProviding ongoing patient care for stabilization of health problems and preventive health care.
oOrdering laboratory and other tests as needed.
oProviding education and counseling to patients and family members.
oScreening patients to determine their need for hospital admission/long-term care.
oPerforming administrative and quality improvement duties, etc.
This position may be eligible for telework.

Monday - Friday, 8:
00 AM - 4:
30 PMPlease Note:
This position requires Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications.
If selected, Applicant will provide proof of current certifications prior to employment.
Only BLS certification which included Automated External Defibrillator (AED) and ACLS certification obtained from either the American Heart Association (AHA) or the Military Training Network (MTN) will be accepted.

Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
